open the attached link and follow the instruction. Since google drive couldn't have the digital pdf, you may need to print it out. However, to access the digital pdf, you may go to your school email account or your parent's where there is an attachment for it; it is a digital pdf that you may fill in the blank and turn it in thru emailing to
me by mrs.duke@gmail.com or printing it
out.
